You may make a report anonymously; however, this may limit the University’s ability to investigate the incident or provide resources to the involved parties.

Roles
Complainant – any person alleged to have been harmed by the conduct reported on this form
Respondent – the individual who is alleged to have engaged in the conduct reported on this form
Reporting Party – the individual submitting the report. This may include individuals fulfilling their mandatory reporting obligations.
Witnesses – any individual who has knowledge of or information about the harassment or discrimination being reported.
