Forms and Handbooks

These resources are provided for students in the 糖心Vlog传媒 Little Rock/糖心Vlog传媒MS Joint Graduate Program in Bioinformatics.

Handbooks

• Bioinformatics Graduate Student Handbook – an information guide and requirements for all students in the Bioinformatics Program.
• Thesis Guide – describes deadlines, procedures, and formatting required for dissertations.

Committee Forms & Sample

• Designation of Advisory Committee Form – for both Masters Capstone and Ph.D. Dissertation/Research Advisory Committees.
• Candidacy Examination Form – to be completed prior to Ph.D. proposal defense and candidacy examination.
• Dissertation Title Page Sample – contains proper wording, formatting, and layout for the dissertation title page.

Program Forms

• Registration and Advising Form – to be completed each semester and approved by the advisor or program director.
• Lab Rotation Report – For master’s students after each lab rotation. Project advisor should email the completed form to the program director, and the student should provide a copy to their advisor.
• Annual Student Progress Report – required to be completed by January 15 each year and should be submitted to the program director.
